Join us for the 2025 Patient Safety & Risk Management Conference! We are grateful for this opportunity to interact with our valued clients in an in-person setting. Copic understands the various pressures healthcare professionals face, particularly over the past several years. We aim to provide you with vital and timely educational material that assists in your daily work. We appreciate and recognize your effort and dedication to the delivery of safe, high quality patient care.
In order to reach as many of our clients as possible, it is our pleasure to present this conference at no cost to attendees. (Please note, this conference is only open to current Copic-insureds.) The conference includes all sessions; Monday breakfast, lunch, and dinner; Tuesday breakfast, lunch, and reception; Wednesday breakfast; and conference hotel rooms for Sunday through Tuesday evenings.
Registration information will be sent via email in January. This conference will be hosted in person; there is no virtual option.

Copic has secured a block of rooms for attendees. In order to guarantee a room for this event, reservations must be made prior to March 28, 2025. A credit card must be provided at check-in for ancillary charges. If you make a room reservation, check in at the Omaha Hilton, and attend the conference as planned, Copic will pay for the room and your credit card will not be charged. If your plans change and you do not cancel your reservation, per the hotel’s cancellation policy, your credit card will be charged.

Description: This session will delve into essential concepts related to patient safety and organizational culture in healthcare settings. Content will include introducing the initiatives of the Nebraska Coalition for Patient Safety (NCPS) and discuss the benefits of collaborating with Patient Safety Organizations (PSOs) to enhance patient safety outcomes. The session will explore patient safety culture, examining its core components and their impact on improving healthcare practices. We will also define Just Culture, highlighting the skills and practices needed to support its successful implementation in healthcare organizations. The session will also introduce systems thinking, exploring how a healthcare system functions and how to design more efficient and effective systems for improved patient care. Additionally, strategies for fostering a culture of reporting, with an emphasis on the role of psychological safety in building a learning culture within organizations will be discussed. Concepts related to building a learning culture and its significance in continuous improvement and patient safety will be discussed. By the end of this session, participants will gain a comprehensive understanding of these key concepts and how to apply them to create safer, more effective healthcare environments.

Description: Though not traditionally seen as part of their professional responsibility, physicians are increasingly becoming advocates for patients and the profession of medicine, often without formal training on how to do so. As such, physicians must embrace this opportunity to advocate for patients and our practices at all levels of influence. Through learning about the requirements for success as an advocate, physician attendees will be able to translate skills from the exam room into action that will influence policymakers across levels of government.

Description: Have you ever thought, "If it weren't for humans, my job would be so much easier?” Most of us have. We learn to navigate complex, constantly changing healthcare systems to accomplish the tasks required of our job responsibilities. That gives us the opportunity to grow and change in sync with the system by responding to the different rules, expectations, and processes as they emerge. We are in a state of constant, if stressful, growth. The challenge is that half of the people involved in this dance of caregiving and care receiving do not have privy to the support of changing knowledge. Patients, whom we expect to be engaged, participative, and cooperative, are simply respondents in the new world of healthcare. How can they be engaged when they haven't a clue of what that means?
In this talk, we will discuss the murky world of patient engagement, what it means today, what it means to patients, and how to truly give patients the tools they need to understand and participate in their care. It may not be what you think.

Description: Falls in healthcare are a significant cause of patient injury with fall rates increasing each year. In this presentation we will look at causes of falls in the environment, ambulatory, and in-patient settings. This presentation will demystify fall myths. After identifying areas of fall risks, we will discuss strategic ways to mitigate falls, prevent patient harm and make all our healthcare environments safe for both patients, visitors, and employees. Takeaway tips will be included in the presentation for the attendees to take back to their respective healthcare settings for utilization.

Objectives:
Description: In this presentation, Copic’s Chief Medical Officer and General Counsel will discuss the trends and recurring issues that are called into the risk management hotline. They will share case examples of real calls and describe how the risk management concerns of insureds have evolved over time. These calls will offer the perspectives of both a practicing physician and a practicing attorney and show the audience how their respective analyses may be similar or different. The presenters will invite audience participation throughout the session and will have an interactive question and answer section at the end where attendees can ask these experts questions from their own risk management concerns.

Description: Root cause analysis (RCA) is widely used in healthcare as an approach to reducing the likelihood of recurrence of adverse events. A well-done RCA should yield robust corrective action strategies and risk reduction. Effective event review and causal analysis requires the active involvement of leadership, training of teams in safety science, focusing on stronger systems-level solutions, implementation oversight and impact on outcomes. This course will describe and review the elements of a well-done RCA and present tools and resources that can be used to ensure the implementation of strong systems-level solutions and oversight measurement.

Description: In this session, key differences between a patient complaint and a grievance will be highlighted. We will review the elements of a patient grievance, the governing law and regulations, the importance of implementing policies and procedures, and necessary steps in drafting a clear response. Carla’s clinical background is in laboratory medicine, and she has a Masters in Healthcare Administration degree. She has worked in a variety of clinical settings including Nebraska Medicine, OneWorld Community Health Centers, Memorial Community Hospital in Blair, NE, and Community Blood Bank in Lincoln, NE. She has held a variety of roles in these various settings: bench tech, department manager, Operations Director, and a Patient Safety Clinical Quality Manager. In all these settings she has been involved in patient care and quality improvement initiatives.
She is a TeamSTEPPS Master Trainer, a Six-Sigma Green Belt, a Zero Harm Trainer, and a Certified Professional in Healthcare Quality.
Emily is a licensed and registered occupational therapist and holds a board certification in gerontology from the American Occupational Therapy Association. She is a graduate of Creighton University with a BS in Health Sciences and a Doctorate of Occupational Therapy. She has an MBA in Healthcare from Nebraska Methodist College and a Black Belt in Lean Six Sigma. Emily has 10 plus years of clinical and leadership experience including program development, operational and fiscal management, and quality improvement. Emily comes to NCPS from Nebraska Methodist College, where she was an associate professor and the founding program director for the Post-Professional Occupational Therapy Doctorate program. Emily has served in leadership roles with the Nebraska Occupational Therapy Association and the American Occupational Therapy Association, she serves on various community guilds and boards, and she is an alum of Leadership Omaha Class 43.
Dr. Warchol is an Assistant Professor and a Staff Physician for the Department of Emergency Medicine at University of Nebraska Medicine. Passionate about advocating for physicians and patients, she has been involved in organized medicine since she completed medical school and residency at the University of Nebraska Medical Center. Dr. Warchol also completed her Fellowship and Master of Public Health through George Washington University in Washington, D.C. She currently serves on the Nebraska Medical Association’s Board of Directors and is active in the American College of Emergency Physicians.
Pamela is the Senior Manager of Practice Quality and Facilities at Copic. She is a graduate of Johns Hopkins School of Nursing. Prior to joining Copic in 2021, Pamela worked as a risk/claims manager for a large health system in Texas and as a risk manager/analyst for a medical malpractice liability company in Arizona.
Dr. Amori has over 35 years of experience in healthcare Risk Management, nine years in mental health care, and more than 40 years in education. She leads Communicating Healthcare, focusing on the psychological aspects of communication for individuals and organizations.
Geri served Coverys/Med-IQ for nearly 15 years, first as Director of Education and later as Vice President for Academic Affairs, educating on risk management and patient safety. She is nationally recognized for her work and has received multiple awards from the American Society for Healthcare Risk Management and is known for her work on communication in healthcare, particularly around difficult conversations, conflict resolution, and supporting healthcare providers.
Matt joined Copic in 2023 as General Counsel and Compliance Officer after more than 27 years in private practice, which included many years of working with Copic on various issues. His background includes a broad range of legal expertise in the areas of business growth, contracts, risk, tax strategy, succession, development, employment, and regulatory issues, including significant experience representing physicians, practice groups, and health care systems. His practice included extensive federal and state litigation and trials in multiple states and foreign countries. Matt received his Bachelor of Arts from Boston College and obtained his Juris Doctorate from Loyola University of Chicago.
Dr. Zacharias was named Chief Medical Officer in 2024 and oversees Copic’s Patient Safety and Risk Management department. He joined Copic in 2015, and previously served as Director of Medical Education. Prior to Copic, Dr. Zacharias held key leadership roles, including Chair of the Department of Internal Medicine and Medical Director of Patient Safety at Boulder Medical Center, Board Chair of Boulder Valley Care Network Clinically Integrated Network and Assistant Clinical Professor of Medicine at the University of Colorado School of Medicine. He has led patient safety and risk management efforts for over 20 years for several organizations, including large, multi-specialty groups, a state-wide consortium of medical groups, and community health systems. Dr. Zacharias is board-certified, earned his medical degree from Vanderbilt University School of Medicine, and completed his internal medicine residency at University of Colorado Health Sciences Center. He is also the author of The Mediterranean Diet: A Clinician’s Guide for Patient Care and the host of COPIC’s podcast, Within Normal Limits.
Mary is a Senior Patient Safety and Quality Specialist at ECRI and the Institute for Safe Medication Practices Patient Safety Organization. She provides patient safety solutions, reviews safety reports, and develops educational materials for healthcare facilities. She is also a Team STEPPS and Just Culture Master Trainer.
Before ECRI, she was Senior Director of Patient Safety, Quality, and Regulatory Affairs at a large health system. She has a lot of experience in patient safety and quality improvement and is a lean six sigma blackbelt. Her background includes failure mode and effects analysis, event investigations including RCA analysis, emergency nursing, care evaluation coordination, and teaching healthcare quality. She is certified in healthcare quality and in patient safety.
Jeraldine is an experienced leader and Registered Nurse with a wide range of skills in healthcare. Her background includes acute and chronic disease management, risk management, and quality initiatives. She has a strong project management experience and has developed tools for patient safety. Jeraldine began her career in acute care and moved to outpatient settings, focusing on creating a culture of safety.
In her current position as Associate Director of the ECRI & ISMP Patient Safety Organization, she leads a team responsible for analyzing the causes of adverse events to improve patient safety. She also promotes education and collaboration in healthcare safety on a global scale. Jeraldine holds a BA, is a Registered Nurse, and has a Master's in Business Administration concentrated in Healthcare Management. Other accomplishments include certifications and several published works.
Dr. Rosenquist is a native Nebraskan, graduated from the University of Nebraska College of Medicine and completed his Family Medicine Residency with University of Nebraska Affiliated Hospitals in Omaha. He is certified by the American Board of Family Physicians and a Fellow of the American Academy of Family Physicians. He maintains a medical practice with Columbus Family Practice Associates PC in Columbus. He was a member of Copic’s Nebraska Claims Committee until beginning his Consultant role in March 2010. In 2022-2023 he served as the President of the Nebraska Medical Association. In his free time Dr. Rosenquist enjoys spending time outdoors and with family.
Dean is the deputy general counsel at Copic, a medical professional liability company in Denver, Colorado. He was previously a shareholder at the law firm Kennedy Childs, specializing in health law, insurance, and business counseling from 1989 to 2013. Born in Denver, he earned a Bachelor of Science in Business Administration and a Juris Doctorate from the University of Denver. His private practice involved representing healthcare providers on various legal matters including entity organization and formation; sale and dissolution; employment and other contracts and policies; regulatory, HIPAA, and antitrust compliance; professional review, privileging and credentialing issues; billing problems including fraud and abuse issues; and a variety of other health care business issues. Dean has experience with insurance carriers, covering disputes and compliance. He serves on the Board of Directors and as the Medical Malpractice Committee Liaison for the Colorado Defense Lawyers Association, on the Board of Directors and Legal Advisory Board for the Colorado Civil Justice League, and on the Board of Directors for Project Angel Heart.
Dr. Sgambati is a Senior Physician Risk Manager at Copic. She attained her undergraduate degree from Brown University with honors in Biomedical Ethics. She is a graduate of New York Medical College, where she was elected to the Alpha Omega Alpha honors society. She completed her general surgery training at the University of Connecticut. During residency, she performed 3 years of cell biology research at Yale University. Dr. Sgambati completed fellowship training in Colon and Rectal Surgery at the Mayo Clinic in Rochester, Minnesota. She practiced at Rose Medical Center in Denver for 22 years, where she was the Chair of the Department of Surgery since 2007 until she joined Copic in 2020.
Joshua has been sharing magic for nearly twenty years and recently appeared on the TV show Penn & Teller: Fool Us, which has gained almost half a million views on YouTube. He was the only magician invited to perform at the World’s Fair in Japan, entertaining visitors for two weeks. Before magic, Joshua was a professional actor, known for his roles in Cheers and Clueless, and showcased his martial arts in Teenage Mutant Ninja Turtles. After 14 years of professional acting, he focused on service work in Latin America, where he discovered his passion for magic. Now, he presents “The Magic of Communication” at at conferences, seminars and retreats throughout the country. And as a magician, he performs his stage show, Joshua Lozoff: Life is Magic at performing arts centers and corporate events across the US, as well as Asia and Latin America.
Dr. Dwinnell is a graduate of Dartmouth Medical School and completed his internship, residency, and chief residency in internal medicine at the University of Colorado. He has a Professor of Medicine at the University of Colorado School of Medicine where he has been a faculty member since 1995. He has been a clinician-educator his entire career and currently serves as the Associate Dean of Student Life, overseeing student affairs and admissions for the School of Medicine. He previously served as the Director of Medical Education at Presbyterian St. Luke’s (PSL) where he also held the role of medical staff president and started the first professionalism committee at PSL. His medical educational interests have been in professionalism, professional development and physician-patient communication.
Dr. Lembitz is the Senior Medical Director at Copic. He has led the patient safety risk management department at COPIC since 2002, in previous roles as VP and CMO. His responsibilities include oversight of the COPIC 3Rs program, the Copic resident education rotation, the Copic CME education library of live and on demand courses, and the daily staffing of the 24/7 physician hotline. He speaks locally, regionally and nationally on disclosure of unanticipated medical outcomes, alternatives to tort, CRP’s, specialty specific risks and case conferences, diagnostic errors, professionalism in medicine, and timely topics in liability, safety and risk. He was a contributor to the AHRQ Candor program development and toolkit. He also authored articles in national journals, including NEJM and Health Affairs, and authored the chapter “Patient Safety Education in Graduate and Continuing Medical Education” in the textbook Patient Safety in Surgery.
Stephanie joined Copic in 2003 in the Patient Safety/Risk Management department. As Senior Manager of Early Intervention Programs, she is responsible for the management of the 3Rs Program and Occurrence Reporting team. She also provides education regarding communication, disclosure and Copic’s 3Rs Program. Prior to her position as Senior Manager of Early Intervention Programs, she was a Practice Quality Reviewer for Copic, where she spent time educating physicians and staff using the Patient Safety/Risk Management guidelines developed by Copic. Stephanie has received training in Mediation and Conflict Coaching and has more than 20 years of experience in patient safety. Stephanie has been a Registered Nurse for more than 30 years with clinical experience in pediatrics and OB.
Andrea is a Claims Supervisor at Copic, having joined in May 2018. She has over 22 years of experience, both in primary and excess exposure, with a total of 34 years of claims management experience. Her responsibilities at Copic include the supervision and oversight of five claim consultants as well as the investigation, evaluation and litigation management of professional medical liability and general liability claims and lawsuits. Andrea has an extensive background in managing complex medical malpractice cases, while also focusing on delivering excellence in customer service to her insureds and other business partners. She earned the designation of Casualty Claim Law Associate and Fraud Claim Law Associate from the American Educational Institute. Andrea earned a Master of Business Administration from Hamline University, St. Paul, Minnesota and a Bachelor of Science degree in Business Administration from the University of Wisconsin, La Crosse, Wisconsin.
Doug brings over 30 years of experience in the management of professional liability claims. This involves the management of complex medical malpractice claims nationwide from both an underlying as well as excess exposure. Doug joined Copic in 2012 and his previous experience included working for Zurich-American Insurance Group managing litigation of over 150 multi-line professional liability claims with an emphasis on medical malpractice nationwide and working with Illinois Hospital Association as a Claims Supervisor. Doug earned a B.S. in Finance from Illinois State University and has the designation of Casualty Claim Law Associate by the American Educational Institute. He also chairs a panel of other medical professional liability companies to regularly discuss industry trends and the effects on the marketplace.
Todd is a Senior Claims Consultant at Copic Insurance Company and brings thirty-one years of experience managing professional liability claims. Prior to joining Copic in June 2022, Todd served as Vice President of Claims and Claims Manager for a successful physician owned medical malpractice insurance carrier. The majority of his experience involves assisting physicians and clinics in medical negligence claims. He has also managed primary facility exposures as well as excess exposures. Over the years, Todd has made numerous presentations regarding claims experiences and risk management issues to clinic managers, physicians and hospital personnel. Todd earned a B.S. in Business Administration and a Master of Health Science from Mississippi College.
JoAnn Bouda is a team building coach with more than 16 years of experience transforming leaders and teams across all industries. She is certified in Conversational Intelligence® and a master coach and trainer from the Academy of Coaching Excellence. JoAnn is an expert at supporting professionals in navigating the difficult leadership, communication, and team challenges that arise in the workplace. As a Professionally Certified Coach with the International Coaching Organization, JoAnn holds the highest level of integrity and prides herself in your exceptional track record and results. She has worked with companies such as Sprint, Hospital Corporation of America, First National Bank, and Blue Cross Blue Shield.

Ms. Comerford is a licensed occupational therapist and certified compliance professional with 20+ years of clinical and managerial experience in a variety of settings including inpatient rehabilitation, home care, subacute/long-term care, and schools. She is presently a senior patient safety and risk management analyst providing services to physician practices through ECRI's Ambulatory Care Risk Management program. Ms. Comerford supports both a nationwide audience and individual clients; she also serves as an analyst for FQHC members of the ECRI and ISMP Patient Safety Organization (PSO). Ms. Comerford guides internal teams and clients in developing novel approaches and adopting fresh perspectives for content development and delivery.
Jennifer is a Nurse Specialist and 3Rs Administrator in the Patient Safety and Risk Management department of COPIC. She received her nursing degree from Pikes Peak State College and joined COPIC in 2019. Jennifer’s clinical experience includes behavioral health nursing and hospice care as a registered nurse case manager and Clinical Director.
As COPIC’s Deputy General Counsel, Mr. McConnell provides legal risk management services and has been a health law attorney for over 25 years. Prior to joining COPIC, he was a shareholder of Kennedy Childs, practicing in the area of health law, insurance, and business counseling. His experience includes health care matters such as HIPAA, business contracts, and fraud and abuse. Mr. McConnell earned his law degree from the University of Denver and currently serves on the board of directors and the executive committee for the Colorado Civil Justice League.
Dr. Rosenquist is COPIC’s Nebraska Risk Management and Patient Safety Consultant and has primary responsibility for conducting educational programs for COPIC-insured physicians in Nebraska. A native Nebraskan, Dr. Rosenquist graduated from the University of Nebraska College of Medicine and completed his Family Medicine Residency with University of Nebraska Affiliated Hospitals in Omaha. Dr. Rosenquist maintains a medical practice with Columbus Family Practice Associates in Columbus and currently serves as the president of the Nebraska Medical Association.
Dr. Sgambati, COPIC’s Medical Director, is a native of northern New Jersey and received her undergraduate degree from Brown University with honors in Biomedical Ethics. She is a graduate of New York Medical College, and completed her general surgery training at the University of Connecticut. During residency, she performed three years of cell biology research at Yale University and completed fellowship training in Colon and Rectal Surgery at the Mayo Clinic in Rochester, Minnesota. Dr. Sgambati practiced at Rose Medical Center in Denver from 1999-2020, was the Chair of its Department of Surgery for 13 years, and co-founded the Colorado Comprehensive Pelvic Floor Center.
Cindy is a COPIC Patient Safety Risk Manager for our Midwest region. She received a Bachelor of Science in Nursing, as well as a business minor from Clarkson College, and has been a practicing nurse for 23 years before joining COPIC in 2018. Cindy provides facility assessments, practice quality reviews, and education to insured facilities and clinics. Her clinical experience includes oncology, home health and hospice, hospital performance improvement, and most recently Director of Quality Services in a critical access hospital.
Heather is a Patient Safety Risk Manager who joined COPIC in 2021, bringing more than 17 years of industry experience. She holds a master’s degree in Healthcare Administration from the University of Phoenix, a bachelor of science in Healthcare Administration from Metropolitan State College, and an associate degree in Nursing. Heather is also a member of the American Society of Health Care Risk Management (ASHRM), Director At Large for the Colorado Healthcare Associated Risk Managers (CHARM), and holds CPHRM, LEAN Six Sigma, and Just Culture certifications.
Dr. Zacharias is the Director of Medical Education at COPIC whose specialty is Internal Medicine. He received his medical degree from Vanderbilt University School of Medicine and completed his residency at the University of Colorado Health Sciences Center. He has served on the Boulder Valley Care Network’s Board of Directors and was previously the Medical Director for the Boulder Medical Center to Front Range Risk Management. Dr. Zacharias is also the author of The Mediterranean Diet: A Clinician’s Guide for Patient Care, published in 2012, and host of COPIC’s podcast, Within Normal Limits.

Copic is accredited as a provider of continuing nursing education (CNE) by the American Nurses Credentialing Center’s Commission on Accreditation (ANCC). A total of 8.5 contact hours will be awarded. You must attend both days for credit; no partial credit will be awarded for this event.
